BlueCross BlueShield Contest Rules

Every new group contract, Individual and Medicare Supp/Select member will be counted, provided they meet the specifications outlinedbelow. A contract is an eligible small or large group employee or individual or Medicare Supplement subscriber accepted by BlueCross and BlueShield of Kansas City. Group, Individual and Medicare Supp/Select contracts will be tallied monthly by BCBSKC Marketing. Group, Individual and Medicare Supp/Select members will be counted separately. The start date is contracts effective October 1, 2005. The end date of the contest is January 15, 2006. Contracts will be counted during the three and half months between October 1, 2005 and January 15, 2006, inclusive. The contest will be administered by Sylvia Surjaatmadja, Manager of Marketing Communications for BCBSKC. All prizes must be claimed by contacting Sylvia Surjaatmadja at sylvia.surjaatmadja@bcbskc.com.

1. Only group contracts for insured Blue-Care, Preferred-Care Blue, Preferred-Care, BlueSaver HSA-compatible plans, PersonalBlue HRA, and Individual/Medicare Supplement/Select members are eligible. Includes Insured, Cost-Plus and ASO. Does not include franchise or PHP.

2. Group contracts, Individual and Medicare Supp/Select members will be tallied SEPARATELY for prizes. Group, Individual and Med Supp/Select members will NOT be mixed to determine prizes.

3. Only Producers are eligible (not agencies).

4. Producers can receive a maximum of 1 prize during this sales incentive program, if a trip is earned from more than one segment, the producer must choose one destination.

5. Only NEW members are eligible, not new contracts from existing groups.

6. Only contracts from groups not with Blue Cross and Blue Shield of Kansas City for the previous 6 months from enrollment date are eligible.

7. Only health contracts are counted (no life, dental, etc.).

8. BCBSKC reserves the right to monitor revenue generated per contract and adjust the production levels required for awards as needed.

9. Any tax implications of a travel award are the sole responsibility of the recipient. 1099 forms will be issued for 2006 tax year.

10. No substitute awards, including cash payment are available. Travel to alternate destinations will not be provided.

11. Any sales submitted but later rejected by BCBSKC for any reason, including health underwriting, are not eligible.

12. Prizes will be awarded January 15, 2006.

13. Travel must be scheduled between January 15, 2006 and December 31, 2006. Travel must be completed by December 31, 2007.

14. Blue Cross and Blue Shield of Kansas City will be the final arbiter in determining producer qualification for award and/or any contest rule interpretations.
